Right ventricle-specific therapies in acute respiratory distress syndrome: a scoping review
This article summarizes knowledge and identifies gaps in evidence regarding treatment of right ventricular dysfunction (RVD) in acute respiratory distress syndrome (ARDS). The authors conducted a systematic review of 51 studies reporting on 1,526 patients with ARDS and RVD. They categorized the studies by type of intervention: ventilator settings, inhaled medications, extracorporeal life support (ECLS), intravenous or oral medications, and prone positioning. They found that most studies were non-randomized experimental studies or case reports with low quality of evidence. They also found that only 14 studies were conducted with the goal of treating RVD directly. The authors concluded that there is a lack of high-quality evidence on how to manage RVD in ARDS and called for more randomized controlled trials to address this knowledge gap.
